Minimum details are requested from you in order that Make the Change can provide you with the requested service. Information collected on registration includes name and e-mail address. If you are to receive counselling, additional information will be required and you must complete a declaration before commencing. If subscribing to the ezine, only your email address is needed.
Records of counselling sessions will be kept. However, these will be stored separately from your client details, to ensure your privacy.
A cookie is used solely for login purposes. There are no third party cookies within the site. You can access and view the majority of this website without revealing any information about yourself. The only exception is you need to be registered and logged in to access services.
For statistical purposes, the server collects and stores the following information about visitors to Make the Change:
- IP address and browser type
- Date and time the site was accessed
- The Internet address of the website from which you linked to the site.
This information does not identify individuals and is used to track the number of visitors to different sections of the site.
Email addresses are solely used to provide the services you request, and are not provided to third parties.
All reasonable steps are taken to safeguard your personal information. Users are required to register and log in to use available services. SSL is used to protect the security of the information passing between the server and your browser. All data is stored securely on the server and is only accessible to the counsellor. No personal information is stored with counselling records. Userplane does not have access to any of your personal details.
Information provided by you will be treated as confidential, whether through email, the website, telephone or by letter. Make the Change will not disclose personal information to a third party.
However, the following exclusions to this confidentiality will apply:
If it is deemed that you may be of harm to yourself, or others, confidentiality may be breached. Confidentiality may also be breached if required by law and records can be subpoenaed. Please note it is mandatory in South Australia to report suspicions of child abuse, as stipulated by the Children’s Protection Act, 1993.
To ensure best practice I undergo regular supervision. During this time cases are discussed, however names and personal details are not disclosed.
As expressly asked by the user, for example, when a referral is requested.
Any disclosure will be the minimal required and will only be done through discussions with you first, unless it is believed you are no longer able to take responsibility for your actions.
It is in your interest that the information you provide is complete and up to date. If your details change, I should be informed as soon as possible.
Due to the nature of online counselling, you are responsible for protecting the privacy of information received as well as any passwords and login information.
During counselling sessions, you should take steps to ensure your sessions are not observed by others.
Counselling sessions are intended for your use only. You cannot share this information with others, as it is tailored to your individual situation and as such is not appropriate for a wider audience. If you wish to share this information in any way, you must obtain consent from Make the Change.
You agree not to alter or edit records of sessions in any way.
For services other than counselling (i.e. Happy Mails), records will be stored for as long as is deemed necessary for its purposes and will be permanently destroyed once it is no longer needed.
Different rules apply for counselling records and these will be held for a period of seven years, after which time they will be permanently destroyed. If clients are under 18, records will be held until the client has turned 25.
As part of the registration process you will receive a confirmation email. This email serves as a reminder of your login details, as well as the procedure for updating your details and as a summary of services now available to you. Communications may take place from time to time as deemed necessary. However you will never receive unsolicited marketing material from Make the Change.
If you have signed up for Make the Change's ezine, you will receive the ezine via email. You can unsubscribe at any time and the link to unsubscribe is found on the email and at the end of the ezine.
The website may include surveys that ask you for feedback on the services provided. This information is used to improve our product or service offerings and to identify potential website and service enhancements. By providing Make the Change with your information or ideas you acknowledge that Make the Change is free to use the same without any compensation to you, for any purpose whatsoever.
Make the Change provides access to the records held about you. You must provide a written request, including your reasons if you wish to view these records. You cannot view the records of another person without their consent.
